Please fill out the appropriate areas and download all of the necessary complete documents.   Your grant will be processed and reviewed for the next grant cycle.  Please do not waste paper or postage by sending anything in the mail!

Please attach an EEO policy that specifically includes and lists sexual orientation and gender identity, and requires compliance with all other applicable federal and local EEO laws. Organizations with non-compliant EEO policies will not be considered for funding.

EEO policy guidelines must include at least the following:

1. Gender identification (an individual's internal sense of one's own gender)
2. Sexual orientation (determined by whom an individual is sexually attracted to and also has the potential for loving)
3. All other categories protected by applicable federal and local laws. We view this requirement as another tool we can use, in addition to grant making, that can help bring about social change. 

Proposal

Proposal should include:
-The problem, need or issue to be addressed by the project
-Outcomes expected
-Population to be benefitted
-Specific goals and objectives
-Project budget and timeline, including narrative where appropriate
-Other sources expected to help fund the project
-Plan to evaluate and report the results achieved
-Qualifications and experience of key personnel and information about the use of volunteers
-Brief description of the grant seeker and statement of mission and activities
-List of current trustees, key administrators, and staff
-And any other information which will help the Foundation understand the merit of the proposal
